I have 20 years of experience building caseloads across different countries, clinics, and stages of business, and train other clinicians to do the same.
I didn’t figure this out because I was naturally good at business.
I figured it out because I got tired of feeling like my schedule had a personality of its own.
Early on, I was doing everything that should have worked. Seeing patients, getting results, saying yes to anyone who booked, trying to “stay busy.”
Some weeks it worked. Most weeks it didn’t.
And I remember thinking, “How am I working this hard and still not making any money?”
I was ready to walk away.
Instead, I decided to try building my own caseload one more time before calling it quits.
And that’s when it clicked.
If I wanted a full schedule, I couldn’t rely on effort, referrals, or hoping people came back. I had to actually understand how a caseload is built.
Over time, I didn’t just figure it out once.
I built six different caseloads across different cities and clinics. I got so good at this I was able to open my own practice, which I grew to multiple locations and sold within 6 years. Over those years I trained my own team on this method helping them build 6 figure caseloads on repeat.
And to be clear, I tried all the usual things first. Posting online, offering discounts, doctor lunches, saying yes to everyone.
It looked productive. It just wasn’t profitable.
The real shift happened when I stopped asking, “How do I get more clients?” and started asking, “Why aren't my clients coming back?”
Because a full caseload isn’t about packing your schedule full of client who don’t need treatment.
It’s about building something that supports both you and your client’s needs.
Once I understood that, everything changed. Not just in how full my schedule was, but in how predictable it became.
